Great Lakes Kinesiology serves certain natural persons residing in the State of California (each, a “consumer”). This Supplemental Privacy Policy for California Consumers explains rights that California consumers may have under the California Consumer Privacy Act of 2018 (“CCPA”). This Supplemental Privacy Policy applies only to California consumers.

 This Supplemental Privacy Policy does not apply to personal information that we process as a service provider on behalf of our customers or that we collect from job applicants, contractors, or employees.

Personal Information

Terms such as “personal information” in this supplemental privacy policy, are being used as defined in the CCPA. You may be required to submit proof of your identity for these requests to be processed. We will use information you provide to us to verify your request. Such information may include your name, home address, social security number or driver’s license.

You can designate an authorized person to make a request on your behalf by either: (1) having your agent send us a letter, signed by you, certifying that the agent is acting on your behalf and showing proof that they are registered with the California Secretary of State; or (2) by you and the agent executing and sending us a notarized power of attorney stating that the agent is authorized to act on your behalf.  Please note that we may still require you to verify your identity before we process a request submitted by your agent.

We will not discriminate against you because you have exercised any of your privacy rights under the CCPA.

Our Privacy Commitment to You

We are committed to protecting your privacy and ensuring the confidentiality of your personal health information. The types of personal health information we collect may include your name, date of birth, health history, OHIP number and records of the care provided to you. We collect, use and disclose personal health information for the following purposes:

  • to provide kinesiology services to our clients

  • to obtain payment for services provided (from you, OHIP, WSIB, your private insurer or others)

  • to teach students and to provide continuing education to our staff

  • to fundraise for our organization (but you may opt-out of receiving fundraising solicitations by contacting us at the address below

  • to conduct quality improvement and risk management activities

  • to comply with our regulatory obligations to the College of Kinesiologists of Ontario

  • to advise clients about special events or opportunities (but we will always obtain express consent to do so)

  • to communicate with other healthcare providers you receive care or treatment from

  • for other purposes permitted by law

We will collect, use and disclose only as much personal health information as is needed to achieve these purposes. You can withhold or withdraw your consent to the collection, use or disclosure of your personal health information by contacting us (details below).
