Great Lakes Kinesiology serves certain natural persons residing in the State of California (each, a “consumer”). This Supplemental Privacy Policy for California Consumers explains rights that California consumers may have under the California Consumer Privacy Act of 2018 (“CCPA”). This Supplemental Privacy Policy applies only to California consumers.
This Supplemental Privacy Policy does not apply to personal information that we process as a service provider on behalf of our customers or that we collect from job applicants, contractors, or employees.

The types of personal health information we collect may include your name, date of birth, health history, OHIP number and records of the care provided to you. We collect, use and disclose personal health information for the following purposes:
to provide kinesiology services to our clients
to obtain payment for services provided (from you, OHIP, WSIB, your private insurer or others)
to teach students and to provide continuing education to our staff
